I authorize The National Catholic Bioethics Center to make equal monthly charges once every month for 5 months to my credit card in the amount of $650.70. I understand that should any charges be declined, that my card will be charged again two weeks from the date of the first charge. Should my card be declined the second time, I will be asked to leave the program. (includes 3% processing fee)
